PROOF PATHS
Approvals and refusals both matter
Approved means the requested action satisfied the required controls and may proceed under the bounded receipt. Rejected means the required proof was missing or the policy did not permit the action.
A serious governance system records both. If only approvals produce evidence, the organization cannot prove why blocked actions were blocked.
