Refusals and escalations need evidence too
A governance system cannot only preserve approvals. It must also preserve why something was refused or held for review. Layer 4 creates signed evidence for approved, rejected, and requires-human-review outcomes.
This matters for audits, incidents, disputes, and regulatory reconstruction because the refused path can be just as important as the approved path.
